XXX We should allow `with_inst` annotations on higher-order terms. compiler/prog_data.m: Add fields to `pred_or_func' and `pred_or_func_mode' items to hold the `with_type` and `with_inst` annotations. compiler/prog_io.m: compiler/prog_io_typeclass.m: Parse the annotations. compiler/module_qual.m: Module qualify the annotations. compiler/equiv_type.m: Expand away `with_type` and `with_inst`.
