compiler/unify_proc.m:
Implement type-constructor-specific index predicates by generating
switches for them directly, rather than generating disjunctions
that the switch detection pass later turns into switches.
This makes the code smaller, and speeds up its later processing
by later compiler passes.
Clarify the comment that explains why having this module generate switches
is OK.
Give some predicates more meaningful names.

Don't abort for switches in predicate bodies. The new code is not right,
but that does not matter, because the constraint-based mode analysis
system has never been completed.
